ICM Entity Management Person form
The Person form allows you to create a new person entity record within an investigative case, and capture as many identifiable details about the person as possible. The person entity will automatically link to the case, with options to link to another case, or another entity. Not all fields are mandatory, but a minimum amount of information is required for the record to be searchable within ICM.

| First Name | First Name of the person associated with the case. |
| Middle Name | Middle Name of the person associated with the case. |
| Last Name | Last Name of the person associated with the case. |
| Preferred contact method | Preferred contact method of the person associated with the case. |
| Phone number | Phone number of the person associated with the case. |
| E-mail of the person associated with the case. | |
| Gender | Gender of the person associated with the case. |
| Height | Height of the person associated with the case. |
| Weight | Weight of the person associated with the case. |
| Ethnicity | Ethnicity of the person associated with the case. |
| Prefix | Prefix of the person associated with the case. |
| Date of Birth | Date of Birth of the person associated with the case. |
| Hair Color | Hair Color of the person associated with the case. |
| Eye Color | Eye Color of the person associated with the case. |
| National identifier | SSN, SIN, NIN, or Other. |
| National identifier Country | Country from which the person's national identifying documents were issued. |
| Driver's license number | Driver's license number of the person associated with the case. |
| Driver's license issuing state | State from which the person's driver's license was issued. |
| Driver's license issuing country | Country from which the person's driver's license was issued. |
| State/Province identification number | State/Province identification number of the person associated with the case. |
| Passport number | Passport number of the person associated with the case. |
| Passport issuing country | Country from which the person's passport was issued. |
| Additional Information | Any additional information regarding the person |
| Information Completeness | Not all fields are mandatory, but a minimum amount of information is required for the record to be searchable within ICM. The information completeness field indicates whether or not that minimum requirement is met. Displays as Partial or Complete. |
| Street | Current street of the person associated with the case. |
| City | Current city of the person associated with the case. |
| Zip/Postal Code | current zip/postal Code of the person associated with the case. |
| Country | Current country of the person associated with the case. |
| State/Province | Current state/province of the person associated with the case. |
| Person Type | The type of relationship that the person has to the case. Subject, Victim, Witness, or Other |
| Source Type | Related case, Investigative task, or Other |
| Status | The status of the person in relation to the case. For more information on the options for person status, see Person status form. |
| Willing to Testify? | Whether the person is willing to testify in court. |
| Relationship description | A brief, written description of the person's relationship to the case. |
