| Number | Auto-assigned number of the assessment. |
| Name | Unique name of the assessment, for example, Assessment for payment service. |
| Description | Detailed description of the assessment. |
| Assessment type | Type of the assessment. You can choose the Smart assessment or Legacy assessment. |
| Smart Assessment templates | Templates supported by the Smart Assessment Engine. You can choose either the Importance assessment template, Impact tolerance assessment template, Importance and impact tolerance assessment template or all of them based on your assessment requirement. |

| Owner | Owner of the assessment. |
| Approver | Approver of the assessment.Note: The approver must be a different user from the owner and assessor. |
| Assessor | Assessor of the assessment. An assessor can be the Operational Resilience user with the sn\_oper\_res.user role. |
| Milestones | |
| Assessment due date | Due date for the assessment in the yyyy-mm-dd format, for example, 2025-12-02. |
| Compose | Private work notes and comments about the assessment. |
| Activity | Auto-recorded log on the activities related to the assessment. |
