Raise a Compliance Request form
Use the Raise a Compliance Request form in the Employee Center to raise a compliance request.
The following table lists the descriptions of the field values.
| Field | Description |
|---|---|
| Name | Name of the request. For example, a request for the Policy Update - Annual medical examination is mandatory for all permanent employees. |
| Description | Brief description of the request. |
| Request type | Type of the request. |
| Priority | Priority of the request:- 1 - Critical - 2- High - 3 - Moderate - 4 - Low - 5 - Planning |
| Requested by | Name of the person who raised the request. |
| Requested on behalf of | Name of the person that you created the request for. |
| Business Unit | Business unit that is affected by the request. For example, Legal. |
| Location | Location where the request was raised. For example, Japan. |
